Maybe you’ve heard about chiropractic care through a friend or did some research online. Maybe you’re just tired of living in pain. If going to the chiropractor is a new experience for you, don’t worry. We’re here to help you shed your pain and get back on your feet. Your first visit to Momentum will be about getting to know Dr. Julie and gives us a chance to get to know you!
We typically request that you print and fill out new patient information prior to your visit, so we can get right to the good stuff - addressing your pain or concerns. Dr. Julie will review your paperwork and discuss your current condition and goals before getting started. After a quick exam and any adjustment needed, we will take a look at the overall results, and, together, come up with a plan of action specifically for you. As your partner in improving your health, we will be with you every step of the way during your treatment.

Financial - please know that we will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. Forms of payment accepted by the office are check, cash, or any major credit card.
Cancellation Policy - If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. A fee may be incurred if a patient does not show up for an appointment without sufficient notice.
